LPA discovered that facility staff failed to report an incident to CCL that occurred a few weeks ago (date unknown), involving Child 1. During interviews conducted, it was disclosed that Child 1 was left inside the Blue Room unattended, with no staff supervision for approximately 5 - 8 minutes. The incident happened around nap time, between 12:30PM - 12:45PM, as staff were preparing the children for a nap. This poses a potential health, safety, and personal rights risk to children in care. 8 of 8/2/24.

Staff members 1 – 4 obtained a criminal record clearance and have been associated to the facility. Deficiency was cleared at the time of visit and no further action is needed. After reviewing the files of Staff members 1 - 4, it was discovered that Staff members 1 - 4 were hired in the beginning of January of 2024 and had been working at the facility prior to obtaining a criminal record clearance. This poses an immediate health, safety, and personal rights risk to children in care.
