This requirement was not evidenced by: The Facility removed the employee from facility immediately and was sent to do trainings and has completed all required trainings assiged. Center provided proof of completions during visit. Based on interviews it was found that the facility did not comply with the cited section above it was determined a child in care had been handled in an innapropriate,unnecessary rough manner by a staff member and was being forced to nap, which did not align with safe, supportive, or developmentally appropriate practices. This action posed an immediate health and safety risk to the children in care. 8

The center did not provide sufficient supervision; child was left behind for approximately 5 minutes and went unnoticed twice. The center failed to provide adequate supervision to meet the needs of children in care this poses a potential health and safety risk for children in care.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. 1 The center has since developed a new plan of transitioing children from outdoor to indoor time through smaller groups. Director held a staff meeting and typed up notes during LPA visit and reminded staff of changes by having all staff read and sign ackowledging changes. Director wrote up plan in both English and Spanish and provided proof to LPA. Based on observations and interviews, the center did not comply with the section cited above. The center did not provide sufficient supervision; child was left outside of classroom on playground.Child was discovered outside after child knocked on the door. Failing to provide adequate supervision to meet the needs of children in care poses a potential health and safety risk for them. 8
101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. 1 The center will develop a supervision plan, review current supervision practices, and implement any new practices moving forward. As a corrective action, the center must submit all training materials and attendance sign-in sheets for staff members who participated in Licensing by August 22, 2025, or sooner, to address the deficiency.
