Based on interview, record review, and observation staff completed face to name check and C1 went to the restroom without staff knowledge. The staff took the other children outside and C1 was unaware the class left and was alone inside the classroom for five minutes without staff supervision which is a potential health, safety, or personal rights risk to the persons in care.

(a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision ... visual observation...This requirement was not met as evidence by: 1 I pulled everyone into office and reminded of supervision and having a staff meeting on May 5, 2026 and go over playground supervison. I will Inform Licensing about meeting and send over sign in/out sheet of who attended. Based on interview, observation, and record review, C1 was playing on the play structure monkey bars when they fell off the bars into the woodchips. Staff S1 & S2 were not in close proximity, they did not have visual observation of C1 and did not witness C1 fall. S1 & S2 learned of the injury after C2 & C3 brought C1 to them which is a potential health, safety, and personal rights risk to the persons in care. 8
