(3) To be free 1 Director will have an all staff meeting and ensure that all past and new staff have been trained on Personal Rights of all children in care. Director will submit minutes and log of all staff that attended the meeting. from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. Based on interviews conducted and LPA observations of items listed on LIC 9099. This poses a potential health, safety or personal rights risk to children in care.

Based on record review, three outlets at the facility contained very high lead exceedance greater than 5.5 ppb. This poses a potential health, safety or personal rights risk to persons in care. 8 Director will notify all parents/authorized representatives of any corrective action already taken at the facility. Director stated she will conduct a follow up water sample as outlined in Written Directives Section 101705 and submit all required documentation to CCL by 11/18/2022.

Personal Rights. Each child receiving services from a family child care shall be accorded dignity in his/her personal relationships with staff, residents and other persons. This requirement was not met as 1 Director will have all employees complete training and submit completion to CCL by poc date. evidenced by staff interviews. On 7/22/2022, staff place hand/fingers on a child’s chin to make the child understand/listen what was being directed to the child. Although, the intent was not to humiliate the child, the dignity of the child was violated. This poses a potential health, safety or personal rights risk to children in care. 8
