Based on observation and interview, the licensee did not comply with the section cited above. LPA observed that the protective coating on the play equipment in both the school age and preschool outdoor play areas was significantly deteriorated. Multiple sections of the equipment showed visible peeling, chipping, and worn surfaces, indicating ongoing wear and exposure to weather which poses a potential health, safety or personal rights risk to persons in care.

Licensee also stated that parents were spoken too regarding incident. Licensee will do training regarding care and supervison and will send a copy of the training and staff signatures to LPA by 09/23/25. This requirement was not met as evidenced that child #1 and child #2 engaged in inappropriate behavior as this incident could have been prevented had greater attention been directed towards supervising the children, rather than focusing on nap preparations which posed a potential risk to the health, safety, or personal rights risk to persons in care. 8
