Based on an unusual incident report and director statement, on 4/17/26, a child left the facility playground without staff being aware at 10:30 a.m. and was found by a teacher from the elementary school that the facility is located at, at 10:34 a.m. which poses a potential health, safety or personal rights risk to persons in care.

Based on a unusual incident notification submitted to the department and staff interviews, an adult (S2) under the supervision of facility staff stepped on a child's (C1) foot in response to that child stomping on S2's foot which poses a potential health, safety or personal rights risk to persons in care. 8 at https://ccld.childcarevideos.org/ and submit to her a statement including what they learned from the video. S1 will then submit the collection of staff statements to LPA by email at robert.maciel@dss.ca.gov by 6/5/25.

Based on two Unusual Incident Reports submitted by the facility and interviews from staff, two children (C1, C2), eloped from the playground into the Calpella Elementary School playground without being observed which poses a potential health, safety or personal rights risk to persons in care. 8 at https://ccld.childcarevideos.org/ and submit to her a statement including what they learned from the video. S1 will then submit the collection of staff statements to LPA by email at robert.maciel@dss.ca.gov by 5/30/25.

Based on an Unusual Incident Report submitted by the facility and interviews from staff, a child (C1), eloped from the playground into the parking lot during a time where moving cars were present which poses a potential health, safety or personal rights risk to persons in care.

Based on record review and interview, the facility did not possess immunization records for S1-S3 which poses a potential health, safety or personal rights risk to persons in care.
