Based on observation, the licensee did not comply with the section cited above in that there was not a carbon monoxide detector available to test, which poses a potential health, safety or personal rights risk to persons in care. Faciity reprsentative stated they have one and it was taken down to replace it's batteries.

Based on observation and interview, the licensee did not comply with the section cited above in that children play in an open area with no fence, which poses a potential health, safety or personal rights risk to persons in care. Facility representative stated they were told to lower the fence, and so they have not used the fence area until it can be fixed.

Facility will conduct a personal rights training to all staff, which will also include the CCLD video on Personal Rights and appropriate communication around children. https://ccld.childcarevideos.org/. Facility will send LPA staff signatures showing their attendance and understanding of the meeting. Interviews revealed that staff #1 raised voice to the children, and on one occasion, used inappropriate word toward children and staff. This poses an immediate health, safety, or personal rights risk to children in care. 8 Deficiency Dismissed
