(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility:(2) Request a transfer of a criminal record clearance as specified in Section 101170(f) This requirement is not met as evidence by: 1 Immediately, Director agrees to have S1 complete an LIC9182 Criminal Background Clearance Transfer Request. Director agrees to submit LIC9182, LIC508, and a photo ID for S1 to CCL by 7/13/22. Based on observation, review of documents, and Directors own admission, S1 has a fingerprint clearance but has not been associated to the facility. S1 began working at the facility 6/24/22. During todays visit S1 was observed to be working in the 2's classroom without an association to the facility, which poses an immediate Health, Safety, or Personal Rights risk to persons in care. 8

101238 Buildings and Grounds (c) All outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard shall be kept free of obstruction. This requirement is not met as evidence by: 1 Director agrees to install a child proof guard along the outside hallway bathroom door and/or find another solution which does not consist of using a trash can to hold the door open. Director agrees to submit proof to CCL by 07/19/22. Based on observation, review of documents, and interview, the Director used a trash can to hold the outside hallway bathroom door open which is an obstruction to the passageway and resulted in a child sustain an injury and requiring stiches, which poses an potential Health, Safety, or Personal Rights risk to persons in care. 8
