Based on observation and record review, the licensee did not comply with the section cited above. While reviewing the sign in/out sheets, LPA observed parents signing out their child/children on the wrong dates and not recording the time of day the child/children was signed out which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. While touring the facility 12:45 p.m. LPA observed 14 children in care. While reviewing the sign in/out sheets LPA observed only 13 children were sign in for afternoon care11/13/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
