Based on observation and interviews the facility did not ensure the supervision of C1 during outdoor activities, which posed a potential Health, Safety or Personal Rights risks to persons in care. Prior to incident Active supervision training was completed on 8/14/25. Director will meet with all the staff regarding active supervision on 9/18/25. Staff meetings will continue to be completed once a month and active supervision will be discussed.

Based on observation, laundry room was unlocked and had laundry detergent accessible to children. Room 2 backdoor on right, was unlocked and lead to unlicensed adjacent recreation room/classroom for Boys and Girls Club. This poses a potential hazard to the health and safety of children in care if not corrected.

Based on interview and record review, Child #1 was left behind on the playground during a transition into the classroom, for approximately 1-2 minutes. This is a potential, rather than immediate hazard to the health and safety of children in care as Child #1 was without supervision, in a contained area, for no more than 2 minutes. 8 trained on the new policy. She provided a copy of the Plan and roster today. No further correction is required at this time.
