(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... This requirement was not met as observed by: 1 Facility Representative states to prevent incidents of child locking themselves in the bathroom, the facility will put a latch or door knob cover on the door to make adult bathroom inaccessible to children and send proof in the form of picture to LPA within 1 business days. Based on interviews, the facility did not ensure the supervision during transition from class to the bathroom, where child was left unsupervised for approximately 1-2 minutes which posed a potential risk to health and safety of persons in care. 8

The requirement was not met as observed: 1 Director's assistant Karina Barrett states she will complete component 3 orientation trainings through child care licensing to review reporting requirements and will submit proof to LPA by 8/29/2025 Based on interviews with staff, unusual incident was not reported to child care licensing department. 8

Based on record review (Guardian Database), staff S1; S3 and S4's fingerprint clearances were not associated to the facility. S1 was employed on 8/11/2023. S3 was employed on 5/27/2025 S4 was employed on 4/15/2024.
Personal Rights-The licensee shall ensure that each child is accorded the following personal rights.. To be accorded safe, healthful.. accomodations.. to meet his/her needs. This requirement is not met as evidence by: Based on LPA's observation on 12/09/2021. 1 The personal rights violation has been corrected based on today's observation. Facility was not following the mask mandates which states that children 2 years and older are to wear face covering/mask indoors in all child care facilities. On 12/09/2021, 48 out of 97 children were observed to be unmask. Violation observed on 12/09/2021 is a repeat violation, $1000 is being assessed. This poses an immediate health and safety risk to clients in care. 8
Child's Medical Assessments Prior to, or within 30 calendar days following the enrollment of a child, the licensee shall obtain a written medical assessment of the child. This medical assessment enables the licensee to assess whether the center can provide necessary health-related services to the child. The medical assessment shall provide the following: 1 Director stated that she will ask the parents to obtain a note from the child's medical professional for the exemption of the child from wearing a mask by 12/20/2021. Additional one week may be allowed due to the holiday. Identification of the child's special problems and needs. This requirement is not met as evidence by that facility representative stated that there are 43 children with mask exemptions but need to obtain a note from a medical professional for their exemption. Director will advise parents that without a medical professional note, the child will have to wear a mask or face covering as they do not meet the exemption criteria.

This requirement is not met as evidence by 1 Facility representation will submit a written statement indicating how they will ensure children are wearing masks indoors to Analyst by 9/22/21. Based on LPA's observation on 09/01/2021, today, interviews conducted and facility's health and safety protocols. Facility is not following the mask mandates, children over 2 are to wear face covering/mask indoor. 19 out of 80 indoor children are wearing a face covering today. Civil penalty of $800 is being assessed for failure to correct the citation issued on 09/01/2021. This poses an immediate health and safety risk to clients in care. 8
All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: 1 Director provided the association transfer documents to Analyst during the inspections and the staff member is now associated to the facility. Director stated that she will add something on the employee checklist to ensure Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement is not met as a staff members who has the required background clearances but is not associated to the faciltiy. Civil penalty of $100 is assess. This poses a potential health and safety risk to clients in care. 8 that a staff is associated prior to working at the facility.
(a) The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy. Director stated that she will meet with the toddler teachers and have a plan in place. Director will submit the five children's updated needs and service plan to Analyst and submit the written plan of This requirement is not met as: There are five toddlers in care that are under 2 years old and non of the children has a modification of needs and service plan. This poses a potential health and safety risk to clients in care. 8 correction to Analyst no than 09/24/2021.
