Based on water testing results and interviews, facility tested over the Lead Exceedance level at 5 sink faucet measuring one at 6.9ppb and the other at 11.0 ppb. Director stated the faucet has been used to fill filtered Brita water pitchers and provided the water to children in the past. This poses an immediate/potential health, safety or personal rights risk to children in care.

Based on water testing results and interviews, facility tested over the Lead Exceedance level at 3 sinks and 2 drinking fountains (faucets) measuring at 8.120ppb, 6.150 ppb, 11300.000ppb, 7.130ppb, 787,000ppb. This poses an immediate health, safety or personal rights risk to children in care. 8 They will not be used until levels are no longer testing in exceedance.

101216 (i)(2) Personnel Requirements: Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement was not met as evidence by: 1 LPA associated staff to preschool program and submitted proof to Director today 10/19/22. Proof will be maintained in staff file. Based on observation and record review. Staff #1 is clear but not associated to the preschool program. This posses an potential risk to the health and safety of the children in care. 8
