Based on observation and interview with the acting director, the licensee did not comply with the section cited above in 1 out of carbon monoxide detectors, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 1 out of 3 persons which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 1 out of 4 children which poses a potential health, safety or personal rights risk to persons in care.

Enhanced refresher training will be provided to all staff with the emphasis on safety guidelines as it relates to responsibility for providing care and supervision for children and expectations from staff. Proof of training or training plan to be provided to LPA by 02/24/2022. Interviews disclosed that when enough staff are not present, the children in room A1 are walked to the outside of the classroom, the teacher or teacher's aid will wait at the door while the child walks to the restroom alone. This poses a threat to the health and safety of children in care.

At the time of this visit this sinks have been repaired. The playground struture and the awning will be repaired. Upon completion of repairs, proof will be submitted to LPA. LPA visually observed the sinks located in the children's restroom is peeling, the hard plastic covering on the steps of the playground structure is torn, and the outside awning has hanging paint/wood that falls onto the children's playground. This poses a threat to the health and safety of children in care.

(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility:(2) Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement was not met as evidence by: 1 Staff associated during visit. Civil Penalty assessed. $100 per day for 2 days. Based on the interview/record review, the Licensee did not meet Criminal Record Clearance which poses an immediate Health, Safety & Personal Rights risk to the children in care. During the inspection LPA observed S1 providing care & supervision and does not have a Criminal Record Clearance associated to the facility. 8
