Based on interviews a student was left unsupervised while transitioning children to the elementary school in the morning. Some school children are unsupervised when going to and from the cafeteria to get breakfast, which poses an immediate health, safety or personal rights risk to persons in care. 8 staff regarding supervision and provided topics discussed at mtg. CM implemented name to face procedure, will work on support secondary staff in the morning, and possible picture system to identify children present to help with transitions.
Based on observation & record review, the licensee did not comply with the section cited above in that 1 out of 4 staff files reviewed did not have a current mandated reporter certficate, which poses/posed a potential health, safety or personal rights risk to persons in care.
