Program Lead took administrative action immediately following the incident. Staff meetings were conducted regarding staff scheduling, children's personal rights, and "if you see something, say something" to administration. Another staff meeting will be conducted on 12/18/23. PL will T1 received an arm massage from C1 and C2 during class. T1 told C1 and C2 that bonus points towards the Student of the Week program would be given either before or after the massage was given. This poses an immediate risk to the health and safety of children in care. 8 schedule a training with DSS about Children's Personal Rights by emailing LPA. Program Lead (PL) advised that staff at the center just completed training on March 1, 2023, which included an emphasis on personal rights and supervision. PL will provide LPA with a list of staff who attended. PL also advised that staff currently address interactions between Interviews revealed that facility staff who should have had knowledge of previous incidents failed to prevent Child #2 from inappropriately touching Child #1 on May 25, 2022. This poses and immediate risk to the health and safety of children in care. 8 children as they happen and will provide LPA with a document outlining how they speak to the children about being respectful of each other’s bodies and what to do if they do not feel safe.

Licensing staff learned that Child#1 was left without staff supervision for approximantely 10 minutes on 3/2/2022. 8 A live document was created and laminated so staff can track transitions with dry erase. A Staff Meeting with topices to include visual supervision is scheduled for 3/9/2022. Interim Assistant Site Supvervisor Rodriguez stated that copies of Meeting Agenda and Sign-In Sheets will be submitted to the Department.
