Based on record review and interview provided by licensee indicating this facility has not conducted testing for lead which poses a potential health, safety or personal rights risk to persons in care.
Observation. 1 Site Supervisor indicated that she had already submitted a work order to the maintenence person on site, but has not obtained status on the repair date. Site Supervisor will follow up and provide a photo of the repaired side board panel by POC date: 11/3/2021 During the inspection of the play yard, LPA observed that there was rotted pealing wood paneling on the outisde of the preschool buidling and was exposing rusted nails. This poses a potential risk to the health, safety and or personal rights in care.
