LIcensee stated that she is in the process of hiring additional staff and is given until 3/28/23 to have qualified staff. This is a repeat violation from 2/16/23 and immediate civil penalty of $250 was assessed today. During an office noncompliance meeting, the Licensee stated there are 6 infants at the facility with no qualified staff present except 3 assistants. This poses an immediately health and safety risk to children in care.
Based on interviewed with the licensee, the copies of the licensing reports have not been provided to parents. This poses an immediately safety risk to children in care. 8 This is a failure to correct violation from 2/08/2023 licensing report and a civil penalty of $700 was assessed.

Director understands must be finger print cleared and associated to the facility prior to returning. Based on observation and interview, Andrea Perez was present on 2/2/23, 2/3/23, 2/6/26 and 2/7/23. Based on LPAs observation and interview conducted, Adult 1, Keta Gonzalez was present in the infant playground did not received a criminal record clearance or exemption and a child abuse index clearance. This poses an immediately health and safety risk to children in care.
Based on observation and interview, the facility failed to meet the staff infant ratio. 1 The DIrector stated will look for a qualified staff to care for the infant and will ensure to have a qualified teacher to meet the staff infant ratio. Director will call the child career to bring in qualified substitute. On 2/3/23, LPAs observed 9 infant with 1 Volunteer. On 2/6/23, LPAs observed 11 infants with 2 volunteers. This poses an immediately health and safety risk to children in care.
Based on observation and interview, staff Keta Gonzales did not have fingerprint clearance and was singularly supervising infant children in care.
101629.1(b) Sign in Sign out The licensee shall require that each child be signed in and out by his/her authorized representative. This requirement is not met as evidecnce by: 1 The Director will ensure to have all parents and authorized representative sign the sign in/out for all children. Director will provide copy of the sign in/out sheet via email by due date. Based on record review and interview, The Director did not have the parents or authorized reprsentaative sign in or out the children. 8
