(1)The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. This requirement is not met evidenced by: 1 Owner states she will submit completed/updated sign oin/out sheets for the current children's in care to LPA by due date. Based on obsevation and interview, during droping off at 9:30am, P1 did not sign in C1, and S1 did not remind P1 to sign in C1; and record reviews, 4 out of 9 reviewed sign in/out sheets for the current children in care have missing parent’s signatures/date/time. This poses a potential risk to the health, safety and personal rights risk of the children in care. 8
101238.2 Outdoor Activity Space (c) Equipment and activity areas shall be arranged so that there is no hazards...This requirement is not met evidenced by: Owner states she will submit piture proof of fixing/removing the broken/hazadous items in the playground, to the LPA by due date. Based on observation and interview, there are cracks on the stairs of a slide in the small playground, 3 broken metal horses laying on the side planter of this playground and S1 confirmed children do play with the horses during outside time. This poses a potential risk to the health, safety and personal rights risk of the children in care. 8
101161 Limitations on Capacity (a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirment is not met evidenced by: Owner states she will submit a statement of understanding regarding Limitation on Capcity regulation and a plan to ensure regulation is being met, to LPA by due date. Based on interviews, P2 confirmed that C2 (8 years old) is at the facility between 2:30pm and 4pm for 2 weeks, and 5 of 5 interviewed staff confirmed that C2 was at the facility for a period of time ranging from 5 min to 2 hours. 8

This requirment is not met evidenced by: Owner states she will submit completed/updated children records for C2, C3, C4, C5, and C6 to LPA by due date. During an office noncompliance meeting, the Licensee stated she thinks there are 15 children present at the facility with 3 assistant and no qualified teachers.
