Based on interview, C2 was left to be filmed by C1 when water was dripping onto C2 for the 30 second duration of the incident. C1 sprayed a water gun at the ceiling initially. 8 by the close of business day (5:00 PM). Additions to the parent contract regarding media consent was also implemented.

Licensee will immediately adhere to CA Dept. of Public Health Guidance on the Use of Face Coverings issued June 29, 2021 and Health and Safety Code Section 1596.885. Licensee to submit a written statement to LPA Stewart via email/FAX of the adherence to current CDPH guidance and Health and Safety Code no later than 9/10/21 at 5pm. On 9/9/21, Director did not ensure the personal rights of persons in care to safe and healthful accommodations and engaged in conduct inimical to the health, welfare, and safety of persons in care, in that children over the age of 2 years and facility staff (S1 and S2) did not wear face coverings while inside the facility, as 8 required by the CDPH Guidance effective 6/15/21, and an individual mask exception did not apply. A Technical Assitance Adisory note was given on 6/7/2021 and an email on 7/2/21 which included the 6/29/2021 CDPH guidance was previously provided to Director. This report has been amended to remove the Civil Penalty cited in error. Amendment approved by Licensing Program Manager (LPM) Maria Mueller on 9/14/2021.
