based on record review and observation facility failed to obtain fingerprint clearances for S1 and must be associated to facility. which poses an immediate health and safety risk to children in care. Civil penalty have been assessed. 1 During today's visit, Center director immediately removed S1 from the facility. Livescan form must provided to the department or S1 must have fingerprint clearance upon report back to work at this facility in order to clear this citation.

based on record review and interview facility failed to report new hired director on June 12. 2024 which poses a potential health and safety risk to children in care. 1 Center director states she will comply with the regulations and will notify the owner to report her new role and all required documents to the licensing department for review by or before 7/17/24 in order to clear this citation.

based on record review facility failed to have staff #2, #3 & #4 immunization records for Pertussis, Measles, Influenza, health report, and TB which poses a potential health and safety risk to children in care. Center director agrees to have all staff complete the required immunization, health report, and TB then submit the licensing department for review by or before 08/16/24 in order to clear this citation

based on the observation facility had children napping without a sheet to cover the cot or mat which poses a potential health and safety risk to children in care. 1 Center Director agrees to review the napping requirement with the staff today then collect all attendee's signature. LPA obtained the document during today's viist.

based on record review facility failed to maintain the license capacity by operating over 5 children during today's visit. 1 Center director shall submit a written statement informing the dept. what action will be taken to avoid being over capacity. Licensee will also submit a schedule listing the days and times of each part time children in care by the due 2/23/24. 2/15/24, LPAs observed 53 children were present and the faciity was license to operate 48 children which poses an immediate health and safety risk to children in care. LPA observed 5 children were picked up during today's visit.

based on record review facility failed to obtain 5 parent's signature upon dropping off the children which poses a potential health and safety risk to children in care. Center director shall submit a written statement informing the dept. what action will be taken to monitor the parents sign in and out and the complete parent's signature for children's attendance on 2/215/24 then submit to the department by the due 2/23/24.
