Based on record review, and interviews, the facility did not comply with the section cited above in that, (S7) was observed mishandling a child (C1), which poses an immediate health, safety or personal rights risk to children in care.
101212 Reporting Requirements. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department.... 1 The Director, Jessica Williams, stated she did not fully understand this regulation. LPA explained the regulation fully at the time of inspection. Miss Williams stated that she will make sure to review the reporting requirement regulation and ensure that anything unusual is reported to the Department within 24 hours. Based on file review, staff interviews, and information gathered during another matter, the facility did not report to the Department about an incident that occurred in August 2025, that involved a staff member (S7) mishandling a child (C1), which poses/posed a potential health, safety, or personal rights risk to children in care. 8
Based on staff interviews, several staff have heard a staff in Owls Room yelling at children in an unfriendly manner. This poses/posed a potential health, safety or personal rights risk to the children in care.

Buildings and Grounds -(1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not by: 1 The Director stated that she will have an exterminator do an inspection and cleanup and sent proof to the Dept by 4/8/25. Based on observation there are rodent droppings near the entrance of the Ladybugs Room by toys, by the classroom door of the Frogs that leads to the outdoor playground which poses/posed an immediate risk to children in care. 8
Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not by: 1 The Director stated she would provide proof of repairs already scheduled and a statement describing the plan for the repairs. Based on observation there are rotting areas of the Ladybugs classroom that are visible on the outside but feel soft to the touch inside between the 2 windows on the North side of the classroom which poses/posed a potential risk to the health and safety of children in care. 8
Based on records review 2 of 5 staff files (S4, S5) were missing immunizations records and 2 of 5 staff files (S2,S4) were missing LIC 503 Health screenings which poses/posed a potential health, safety or personal rights risk to children in care.
Based on Director Statement and file review Staff #1 did not complete Mandated Reporter Training as required which poses a potential health safety risk to persons in care.
