The Vice President of Operations Jenni Grawvunder states she will conduct an all staff meeting regarding the personal rights of children in care. The Vice President will send a copy of the signed meeting agenda to the department by 5/26/26. During staff interviews, it was discovered that previous staff member (F1) would yell and make inappropriate comments towards children in care. These actions constitute a failure to treat children with dignity and respect. This is a potential health, safety, and personal rights risk to children in care.
Based on staff interviews, it was disclosed that previous staff memeber (F1) was observed calling children in care "ugly" and yelling statements like "shut up" and "be quiet". LPA determined that this is a potential health, safety, and personal rights risk to children in care.
Based on staff interviews, it was disclosed that former staff member F1 would wear electronic ear buds and/or ear plugs on duty to muffle the sound of crying children which poses a potential health and safety risk to children in care.
Buildings and Grounds (a)(1) The facility shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met by: 1 The Director, Jessica Williams, stated that she will create a checklist for the facility's classrooms, that requires teachers to ensure that there are no sightings of rodent droppings throughout the classroom prior to the childrens arrival. Based on observation, the LPA observed rodent droppings throughout the infant classrooms, which poses/posed a potential health, safety, or personal rights risk to children in care. 8
Based on file review and staff interviews conducted by LPA, there was a staff member caring for children under the influence of marijuana. This poses/posed a potential health, safety or personal rights risk to the children in care.
Based on observation and interview, the licensee did not comply with the section above, as one infant staff did not have a tuberculosis on file and was unable to provide proof of one conducted.
Buildings and Grounds -(1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met by: 1 The Director stated that she will have an exterminator do an inspection and cleanup and sent proof to the Dept by 4/8/25. Based on observation LPAs observed rat droppings in the crib area which poses/posed an immediate risk of healthand safety of children in care. 8
Buildings and Grounds -(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met by: 1 The Director stated she would provide proof of repairs already scheduled and a statement describing the plan for the repairs. Based on observation there is broken/rotting baseboards behind a rocking chair in the infant room which poses/posed a potential risk to children in care. 8
Based on investigation interviews, staff are washing infant bottles in classroom sink with dishwashing detergent but not sterilizing as required which poses/posed a potential health, safety or personal rights risk to children in care.
Based on observation, LPA observed both diaper changing table vinyl cushions were torn with foam filling exposed which poses/posed a potential health, safety or personal rights risk to children in care.
Based on investigation interviews, partially used prepared infant formula bottles have periodically been saved and refed/offered later in the day. Per CDC, "Throw out any infant formula that is left in the bottle after feeding your baby. The combination of infant formula and your baby’s saliva can cause bacteria to grow." By feeding left over prepared formula this poses/posed a potential health, safety or personal rights risk to children in care.
Based on investigation interviews, infant who can hold their own bottles should be placed in appropriate seating equipment to feed but are often placed on their backs and provided a bottle if they can support it on their own which, poses/posed a potential health, safety or personal rights risk to children in care.
Based on observation and interviews, infant room sinks does not deliver hot which poses/posed a potential health, safety or personal rights risk to children in care.
Based on LPA's record review, the licensee did not comply with the section cited above in 2:5 infant files did not include the Individual Infant Sleeping Plan which poses a potential health & safety risk to persons in care.
Based on interview and review of relevant documentation, staff had knowledge of an injury occuring to Child #1 while in care, but had not been directly observing the child when the injury occurred, which could have prevented or explained, what occurred. This is a potential risk to the health and safety of children in care.
Based on interview and review of relevant documentation, Staff #1 observed that Child #1 had an injury that needed assessment. This injury was not reported to the child's parent or recorded in the child's record. This is a potential risk to the health and safety of children in care.
Based on interviews, unqualified staff have been left alone without direct supervision of a fully qualified teacher, assistant director or director, on one or more occasions, during the lead teachers' lunch break while children were awake and at closing, after the lead teachers have gone home. This is a potential risk to the health and safety of children in care. 8 be done during tomorrow's staff meeting and documentation will be provided by the plan of correction date.
Based on Director statement and interviews, Facility representative did not follow covid guidelines as covid positive result of staff #1 was not reported to the Department verbally within 24 hours or in written form within 7 days, which poses a potential health and safety risk to children in care.
Based on observation, interview and review of relevant documentation, the facility was using a play climber that was not age appropriate for children under 18 months. Child #1 was 17 months at the time of the incident. This is a potential hazard to the health and safety of children in care.
101427(c) - The infant shall be fed in accordance with the individual plan. This requirement was not met as evidence by: 1 The child is no longer enrolled. The director states going froward the needs and services plan will be followed. Based on record review and interviews, a 4 month old infant was offered water which was not included on the infants needs and services plan. This posses a potential health and safety risk for children in care. 8
