Based on interviews with staff, the facility failed to report an unusual incident involving Child (C1) who exited the classrom and was found on the playground without staff supervision, to the Department as required which poses a potential health, safety and personal rights risk to children in care.

Based upon facility records, and interviews with staff, the facility failed to provide Child's (C1) authorized representative with a report of the incident involving Child exiting the classroom and being found at the playground without staff supervision which poses a potential health, safety and personal rights risk to children in care.
