Based on Director and parent interviews, it was determined that due to S1 lack of supervision, child had the time to reach the hot lamp on top of a Bearded Drangon's tank and burn his left hand.
Based on the information provided by the Director Kate Yohe, a 5 year old child was left alone and unattended on the playground for approximatley 15 minutes by S1 and S2 on 05/16/2025.
Based on the information provided by site director Kate Yohe, a 2-year old child wandered away from the facility without supervision from staff. 8 Facility will also install a door alarm to sound off whenever the door is opened.
