Based on interview and record review, the licensee did not comply with the section cited above 2 of 2 infant files are missing signed Acknowledgement of Licensing Report LIC9224 dated 8/13/24, which poses a potential health, safety or personal rights risk to persons in care.
(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license... This requirement was not met as evidence by: Based on staff interview, 1 Office Admin stated they will have a plan to prevent comingling from happening. Admin will train staff and review staff schedules and review parent contract 3 staff stated, there are days in the morning that infants and preschooler are together in one classroom until another staff comes in. S2 stated on 8/7/24 and 8/8/24, preschool and infant was together in 1 classroom. This is an immediate risk to health and safety of the children in care. 8 HSC
Based on record review, the director’s documents is missing 8-hr health preventative course with Lead. It is therefore concluded that the director was not qualified. This poses as a potential risk to health and safety of children in care
Based on LPA's observations the infant classroom was out of ratio being 7 1 Admin Binbin Zhao stated they are recruiting 4 new staff in order to keep the facility in ratio. Admin Zhao stated if the facility is understaffed, they will call parents to drop off at a later time. A copy of this action plan will be sent to LPA by due date of 7/10/24. infants with 1 fully qualified teacher at 8:50am. Based on staff interview, S1 stated they are sometimes out of ratio in the infant room. This poses an immediate risk to the health, safety or personal rights to persons in care.
Based on LPA's interviews and record review the infant classroom has had many incidents of being out of ratio when all enrolled children attend and ongoing staffing issues. This poses an immediate risk to the health, safety or personal rights to persons in care.
Based on LPA’s record review and interviews, infant classroom aid was left alone caring for four (4) infants from 4:00pm-5:45pm on 04/05/2024. This poses an immediate health, safety or personal rights risk to persons in care.
