Based on observation and interview, the licensee did not comply with the section cited above as there were presence of pests in the facility but measures have been increased in the past month, which poses a potential health, safety, and/or personal rights risk to persons in care. 8 LPA determined the information obtained is sufficient to clear the citation. Moving forward, Director will ensure continued increased measures will be taken with the goal of keeping the facility free of pest.
Based on LPA observations and interview, the licensee did not comply with the section cited above as a child was left unsupervised in the yard, which posed an immediate threat to the health and safety of the child in care. 8 LPA shall endorse the facility the Technical Support Program (TSP), which is the non-enforcement service of Community Care Licensing to help providers.
, LPA Record Review, Unusual Incident Report (UIR) LIC 624 dated 5/17/2024, showed that on 5/9/2024 at 10am, C1 left unattended in a small room outside of C1's classroom corridor in between classroom and outdoor space for 2-3 minutes which pose an immediate risk to the health, safety or personal rights to children in care. 8 LPA shall endrorse the facility the Technical Support Program (TSP) is the non-enforcement service of Community Care Licensing to help providers, and to the local Resource and Referral (R&R).

Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed 5 staff members have an expired mandated dated reporter certificate. Director stated that staff members forgot that it needed to be renewed.
Based on interviews with staff and parents children are not being treated with respect which poses an immediate health, safety and personal rights risks to persons in care. 8 The plan of correction is for Director to submit a list of signatures of all staff who attended the meeting, a meeting agenda, a new handboook created for staff on acceptable ways to discipline children. These documents must be submitted to LPA via email no later than 10/31/2022 at Sabina.Dodoo@dss.ca.gov
Director terminated the teacher responsible for supervising C1. Director held a staff meeting on 4/13/2022 reviewing regulations regarding supervision of children. On 4/11/2022, a child was left unattended for a brief period before being noticed by a parent. This presents an immediate risk to the health and safety of children in care. 8 Repeat violations will result in immediate civil penalty assessment.
Director produced paperwork detailing disciplinary actions taken against the teacher who acted inappropriately towards daycare children. Deficiency cleared. Director stated that facility teacher forced a child to leave a sleeping cot and used inappropriate tones of voice (described as "harsh" and "punitive") towards daycare children. This poses an immediate risk to the health and safety of children in care.
