Based on record review, the licensee did not comply with the section cited above as one staff on site providing care and supervision to children did not have a fingerprint clearance or exemtpion, which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as 2 of 5 staff files reviewed by LPA did not have proof of immunizations for MMR (measles) and Tdap (pertussis), which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as 1 of 5 staff files reviewed by LPA did not a LIC 503 Health Screening on file. In addition, 2 of 5 staff files reviewed by LPA had TB test results that were from more than 1 year prior to emploment at the facility, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA observation and record review, the licensee did not comply with the section cited above as one child's prescription medication did not have written approval and instructions on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
