Based on record review and interview, the facility did not comply with the section cited above in for mandated reporter training to be completed every 2 years which poses a potential health, safety or personal rights risk to persons in care.
Based on the information obtained during investigation interviews with staff and parents it is determined that there have been occasions infants under 12 months have been permitted to sleep on their sides or stomach prior to their LIC 9227 section C completed which poses/posed a potential health, safety or personal rights risk to persons in care.
