Based on observation, staff and parent 1 Director will meet with the Corperate Office of ChildTime Children's Center, submit a solution on how the CCC plans to meet the needs of children in care to CCL by the end of business day on 08/28/2026. interviews, the classroom identified as the “Twos Classroom” meets the preschool age ratio of 1:12; however, the children’s needs are not being met due to diapering, potty training while also attempting to manage socialization and behavioral challenges.

Based on LPA’s review of facility records, 1 Director will have a staff meeting with all individuals identified to act in the Director's absense, the reporting requirements to CCL and parents/authorized representatives of communicable disease outbreaks. staff and parent interviews, there has been two recent outbreaks of HFMD within the facility and only the parents of the specific classroom that the outbreak occurred were notified. Children share the facility, playgrounds, and staff float from classroom to classroom. This poses a potential risk to the health, safety, and person rights of children in care. 8 Director will submit the agenga and sign-in sheet for "Reporting communicable diseases" to CCL by the end of business day on 08/28/2026.

Based on LPA observation, there was an uncovered outlet, exposing the socket and there is chipped paint at the base of the wall in the classroom within reach of the children. This poses a potential risk to the health and safety of children in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. Based on interviews and records review, day care did not have 1 Director will create a plan to asses any future incidents and within 30mins that it occured, the facility will have the issue fixed or make the decision to close the center for the health and safety of the children in care. water for children in care as stated on LIC 9099.
