Based on record review and LPA observation of surveillance cameras, Staff #1 removed the infant from the chair, and abruptly pulled the bib off from the infant's neck upward over the child's head, startling the infant and causing the infant to wake up and cry. This poses a potential risk to the personal rights of children in care. 8 A copy of the agenda and sign in sheet was provided to LPA.
Based on interviews and LPA observation, on 05/30/2025, Staff 7 was observed spanking Child 3 once on the buttocks, lifting the child, turning the child around and sitting the child down. In addition to spanking a child, the same staff was also observed inappropriately handling Child 4 in care by dragging the child by the feet and picking up Child 4 from the ground by their wrists/forearm.
The licensee shall ensure that each child is accorded the following personal rights: To be accorded dignity in his/her personal relationships with staff and other persons. This requirement is not met as 1 Director stated she will condct an all staff meeting on how to handle a child that is not wanting to sleep and allow them to have a quite activity while the other children sleep. evidenced by: Based records review this incident occurred with staff member using exessive force to have child sleep. This is an immediate health & safety risk to children in care. 8
