Based on record review, the licensee did not comply with the section cited above in that 4 out of 4 staff (Staff 1 [S1], Staff 2 [S2], Staff 3 [S3], and Staff 4 [S4]) files did not contain proof of completion of Mandatated Reporter Training which satisfies AB1207. This poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in that 2 out of 4 staff files (Staff 3 [S3] and Staff 4 [S4]) did not contain the majority of documents from the following sections: California Code of Regulations (CCR) Section 101217, CCR Section 101216 (including CCR Sections 101216(g) and 101216(i)), and CCR Section 101216.2. Based on observation, interview and record review, the licensee did not comply with the section cited above. Drinking water outlets within the facility have not been tested for lead contamination and are still in service and accessible to children in care which poses a potential health, safety or personal rights risk to persons in care.
