(b) The Department has the authority to interview children or staff without prior consent. Licensee shall submit a written statement acknowledging understanding of the Department's authority to enter and interview children or staff without prior consent and agreeing to permit access during future visits. This requirement was not met as Office Administror did not comply with the section sited above in not allowing LPA to interview staff and/or children, LPA waited 25 minutes until Administrator arrived,which poses a safety or personal risk to children in care. Licensee will pay immediate Civil Penalty of five hundred dollars ($500) for immediate violation..

Based on LPA's observation, the licensee did not comply with the section cited above. During a tour of the bathroom, LPA observed that the bathroom counter had disinfected out on the counter and was not put up out of a child's reach. 1 Per Director Staff will lock up all cleaning agent and/or store them on higher shelf making it inaccessible to children in care Upon entering bathroom LPA Dowling noticed that chemicals were on the counter, within reach of children. Additionally, LPA Dowling and LPM Navarro observed chemical's on counter on 5/28/25 which were a risk to persons in care. 8
Based on LPA's observation, the licensee did not comply with the section cited above. During a tour of the bathroom a can of Lysol was on the Bathroom Counter within reach of children it is essential that hazardous items be made inaccessible to children.
101229(a)(1) Responsibility for Providing Care and Supervision (a)The licensee shall provide care and supervision as necessary to meet the children's needs.(1)No child(ren) shall be left without the supervision of a teacher at any time... 1 Director stated they will continue to ensure and monitor adult/child ratio. Director will implement a training where staff will provide verbal notification when walking away from their group. Director will email, mail or fax training sign in sheet to LPA by POC date This requirement is not met as evidence by: Staff 1 who was responsible for child 1, stated she stepped away and did not know how child got bit, but knew it was one of the animals. Other staff reported they did not know how child 1 was bitten resulting in a lack of supervision which poses an immediate health, safety or personal rights risk to persons in care. 8 Amended report supersedes the prior document. (a) The licensee shall ensure that each child has an individual permanent or portable storage space for his/her clothing, personal belongings and/or bedding This requiremnt has not been met as evidence by 1 The facility will provide plastic container for storage of chldren personal belonging and label them with children names. The faility will maintain all personal belongs in cubies labled with each children names. The cubies will provide storage for bedding items. Facility will do a survey to discuss if face mask will continue to be required. Based on staff and parents interviews that children face mask are not being stored properly and being worn by children that mask do not belong to. LPA's observation of mask on floor and in cubies, which posses a potential Health, Safety, or Personal Rights Risk to children in care. 8

101170 Criminal Record Clearancee) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working..in a licensed facility(3)Request and be approved for a transfer of a criminal record exemption... be present at the facility. This requrement was not met 1 Staff member will not return to facility until the criminal background clearanc has been processed. Licensee will continue to process clearance through Guardian. Licensee will contact the deparement when staff member has been associated with the facility. Based on review of records LPA Thompson determined the facility did not confirm the request for a transfer of a criminal record exemption completed and approved, which poses an immediate Health & Safety risk to children in care 8
