On 07/21/2026, C1 was left unsupervised on the playground for approximately 2 minutes while the rest of the class went to their classroom which posed an potential risk to the Health, Safety or Personal Rights to children in care. 8 Site Supervisor Doss also stated they will be reviewing their procedures such as headcounts, doubling back to the playground and other areas when transitioning, and the importance of active supervison at their upcoming All Staff Meeting. Proof of the meeting via attendance sheets shall be submitted to LPA Hollinger by 08/27/2026. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $1000 per violation and $100 per day until corrected.

A copy of the plan shall be submitted to CCL by COB 4/11/25. abuse or other actions of a punitive nature...This requirement was not met as eviendenced by: Staff grabbed a child by the shirt (collar) which is an immediate risk to the health and safety of children in care. 8
