Based on observation and interview, the licensee did not comply with the section cited above in that LPAs observed one door leading from the hallway inside of the building directly to the indoor pool that is not self closing and does not have a key card or code. In addition, there were three (3) exterior doors that lead to the indoor pool and spa and are not self-closing which poses an immediate health, safety or personal rights risk to persons in care.
