This requirement was not met as evidence by; LPAs observed that three areas of the fence, surrounding the 1 Director stated that area's of the fence will be fixed and that proof of correction will be submitted to the department by 4/05/2024. Director stated that rubber flooring will be corrected and proof will be submitted to the department by 4/05/2024. Director stated that sand box will be filled with sand or cushioning playground was loose on the bottom, that a child could fit under and possibly get out. LPAs observed that the rubber flooring is uneven and can pose a tripping hazard. LPAs observed that the sandbox was hard and bare, and provided no cushioning.
