Based on record review the licensee did not comply with the section cited above. In reviewing the sign in/out logs, Child #4, was present and not signed in. This poses/posed a potential health, safety or personal rights risk to persons in care.

In reviewing children files, Child #1, #2, #3, and #6, they were missing an Acknowledgement of Receipt of Licensing Reports (LIC9224) for a Type A citation that was issued on 10/11/2023. Based on observation, the licensee did not comply with the section cited above. While LPA was touring the facility, a child was in the restroom, that is located outside of the classroom, in the hallway, while staff #3 was walking back to the classroom. The stall door was also closed and visual supervision was not possible. This poses an immediate health, safety or personal rights risk to persons in care.
