Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review immunizations for S5. S5 had an exemption on file, however, the exemption is not valid due to it not being signed by a California Doctor.
Based on record review, the Director did not comply with the section cited above in 10 out of 12 which poses/posed a potential health, safety or personal rights risk to persons in care.
