Based on observation, the licensee did not comply with the section cited above in that in all 6 classroosm there was a cabinet present that stored items that stated "call poison control" on their labels. These items were in a storage area that was inaccessible to children. However, the storage area was secured by something other than a key or combination lock specifically. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review and interview, the licensee did not comply with the section cited above in that 2 out of 3 children's files reviewed that required a verification of receipt, did not have one present. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview and video footage obtained, Facility self reported a Staff member inappropriately handed a child in care. This ia an immediate health and safety and personal rights risk to children in care. 8 service and personal rights of children.Documentation of this training and staff attendance will be sent to LPA by POC date.

Admission Agreements: The licensee shall comply with all terms and conditions set forth in the admission agreement. Based on interviews and records review, the licensee did not comply with the section cited above. In reviewing facility files, there was no proof that the facility informed the parent/legal 1 Licensee has conducted meetings and training with the entire staff. the observation made by the LPA that staff Kathia Gonzales was providing care and supervision to children in care however, 1 Licensee immediately submitted documentation to have staff associated to the facility. Licensee and/or Director will submit a statement regarding how the facility will assure compliance with the cited regulation. staff was/is not associated to the facility. This poses an immediate risk to the health, safety, and personal rights of the children in care. This citation comes with a $500.00 civil penalty, which will be issued today 8 Statement to be submitted to the Riverside Child Care Regional Office by 9/2/2022.
