Based on observation, the licensee did not comply with the section cited above. LPA observed multiple tripping hazards in the outdoor play area. There are tree roots that are uncovered and ground is uneven. Sidewalk is lifted and there are mutliple cracks on sidewalk, which poses a potential health, safety or personal rights risk to persons in care.

Upon inspection of 4 staff files, it was observed that staff either had expired Mandated Reporter Training or not available in the file for review.
