... (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time…. Supervision shall include visual observation. This requirement was not met as evidence by.... Director stated they have now emplemented a huge training with a face to name and do it in the classroom every 30 minutes and during transitions every teacher that encounters the group of children as well and the director also does supervision when transitioning. Based on three of three interviews and records reviewed, C1 was left alone in an outside restroom for approximately fifty (50) minutes without supervision. C1 was found by D at the time of pick up after searching the facility with C1’s guardian, which poses an immediate health, safety and/or personal rights risk to children in care. 8 A $500 CIVIL PENALTY WAS ASSESSED

Based on a review of facility records, LPAs did not find Staff 1 associated with the CCC. Records indicate that Staff 1 was associated with the CCC on January 15, 2025, and disassociated on August 26, 2025, despite leaving and returning as an employee at the CCC since Janurary 2026.

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed five large holes in the unlevel hills of the playground. Due to weather the ground is erroded and the holes are repeatedly exposed. Which poses a potential health, safety or personal rights risk to persons in care.
