101239 Fixtures, Furniture, Equipment and Supplies (e) (4) All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition. This requiremnt was not met as evidence by....... 1 By COB director stated, they will write a declaration stating that they will not maintain more than 15 children in classroom #6 and understands there should be one properly working toilet and hand washing station per 15 children present. Based on observation and interviews classroom # 6 had 19 children present on the date of the visit with only one working toilet. Interviews disclosed most of the time children are to dispose of used toilet paper in the trashcan for both urination and bowel movements. Which poses a potential health risk to children in care. 8

Based on observation and interviews, it was revealed at times during nap time, only one staff member will be available providing supervision for more than twelve children during nap, while not all children are asleep or on their cots. On two occasions, LPA observed three of four classrooms out of ratio.
Based on observation, the licensee did not comply with the section cited above incabinet under sink is broken/could not be locked during time of inspection. Bleach cleaner is stored in the cabinet. This poses a potential health, safety or personal rights risk to persons in care.

Director Julia Fletes stated she would submit a plan to ensure if children are not napping during nap time, the class is still in appropriate ratio. Interviews revealed at times during nap time, not all children remain napping or in cots, with only one staff providing supervision for up to 24 children.
