Based on interviews with staff, child-on-child bitings occured when staffs' attention was directed toward the child requiring additional support and adequate supervision of other children was not maintaine which is a potential health, safety and personal rights risk to children in care. 8 The facility will provide LPA documentation proof of the supervision plan and corrected measures to prevent future incidents and will also provide proof of plan to prevent any future incidents and will email to LPA by 6/30/26
Based on interviews and record review, the licensee did not comply with the section cited above. On 2/24/2025 two infants napping in the crib area were without visual supervision when 5 staff and 10 infants evacuated when the fire alarm went off. This deficiency poses an immediate health, safety or personal rights risk to persons in care.
