Responsibility for Providing Care and Supervision:(a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1)No child(ren) shall be left without the supervision of a teacher at any time... This requirement was not met as 1 Director stated that they had conducted a Transitioning and Name to Face training with all staff on 9/29/25 and 10/13/25. Director provided copies from training to LPA during visit. Based on interviews conducted 6 of 6 staff members confirmed that P1 stated they brought C1 into the classroom from the hallway where they were alone. Witnesses interviewed stated they found C1 in the hallway alone and took C1 back into their classroom. This is a potential health and safety risk to the children in care. 8

Licensee will submit pictures as proof of correction on or before the Plan of Correction due date of 09/30/2022. LPA King observed that the restroom inside the infant classroom was being used as a storage, housing various items such as personal belongings, sleeping cots, cleaning tools, play materials/equipment, and other hazardous items, which poses a potential health, safety or personal rights risk to persons in care.
