Based on observation and interview, the licensee did not comply with the section cited above. While touring the facility at 8:58 A.M. LPA observed a bucket filled with used green solution chemical near the entrance of the main building for classrooms M111, M114, and M117 which poses an immediate health, safety or personal rights risk to persons in care.
Based on observation and interview, the licensee did not comply with the section cited above. While touring the facility at 8:58 A.M. LPA observed three storage sheds that were unlocked. Inside the sheds, LPA observed a saw, hammer, screw drivers, wood sealer, paint, WD40, and janitorial supplies that read "WARNING Keep Out of Reach of Children." The sheds are located near the entrance of the main building building. LPA observed the gate separating rooms Y100, Y105, Y107, and Y109 from the main building was open which poses an immediate health, safety or personal rights risk to persons in care.
LPAs observation of living spiders, webs and spider eggs observed on the playground equipment where the children play daily. This poses a risk to the children in care. 8 to be free of insects weekly and to keep record at the facility then submit the plan to LPA by specified date of 09/30/2022.
