The LPA observed no LIC9227 for C2. LPA inquired with the director, the director stated C2 had not LIC9227. C2 is over 1 year old but started at the day care before age 1. Therefore, an LIC9227 is/was required.

(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. 1 LPA observed Site ID P located inside Toddler A Classroom had the faucet portion removed. Facility provides filtered water through Brita water pitchers for children in care. Based on lab report analysis water from outlet P (6.400 parts per billion) taken on 9/17/2022 indicated levels of lead in exceedance. 8
