Based on observation and record review, the licensee did not comply with the section cited above in 1 out of 14 staff (substitute Isabelle Cannon) has the required background clearances but she is not associated to the facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and record review Albert Torres was providing care and supervision to children on 3/16/23 without a criminal record clearance or association to the facility. This poses an immediate health and safety risk to the children in care.

Based on water testing results and interviews, facility tested over the Action Level Exceedance level at one water faucet, multi-use/food prep/drinking faucet-C. This poses a potential health, safety or personal rights risk to children in care. 8 While the faucet was inoperable children filled their water bottles with bottled water and food was prepared with bottled water. The deficiency has been cleared.
