... 1 Director will provide copies of all LIC9224 acknowledgement forms (related to the 11/15/24 licensing report) and the facility roster to LPA Poindexter by 4/1/25. Director will also submit a written, dated and signed statement that, in the future, they will provide copies of any Type A citation and LIC9224 form to parents. Based upon interviews and records review, the facility delayed required dissemination and collection of LIC9224 forms, which provide proof that licensing reports were received. Based on investigation interviews and relevant documents reviewed it is determined that child C1 sustained an unexplained injury at the facility on 10/24/24, due to a lack of supervision. Investigation interviews determine staff S1 observed the child without injury at 5:06pm but observed the child with injury by the time the child was picked up which poses/posed an immediate health, safety or personal rights risk to children in care. 8 and have the staff review the video by 11/20/24 and provide the Department proof with sign-in sheet they reviewed video.

Based on observation, the licensee did not comply with the section cited above. Two children's Epi-pens have expired (Room#5 & Room #6). This poses a potential health, safety or personal rights risk to persons in care.

Based on water testing results and interviews, facility tested over the Action Level Exceedance level at 4 faucets/fountains, 3 of which were drinking fountains. This poses a potential health, safety or personal rights risk to children in care.

Licensee did not provide notifications to day care childrens' responsible parties as required.
