Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time except as specified in sections 101216.2(e)(1) and 101230(c)(1). This regulation was not met by: 1 Director Rosalva stated her staff will be provided additional training on “supervision” to prevent similar incidents from reoccurring. Proof of the training shall be submitted to LPA by or before April 12, 2024. On March 19, 2024, a child was unsupervised in classroom (26) for approximately one (1) minute. Based on a record review that was self-reported via an unusual Incident report dated March 25, 2024, facility staff failed to maintain 100% supervision of C1. This posed an immediate risk to the health, safety, and personal rights of the child in care. 8 A copy of section 101229 Care and Supervision was provided.

All required documentation is to be submitted to the Fresno Community Care Licensing Office by 2/09/2023.
