California Lead Action Level at Child Care Centers (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up... (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. .This requirement was not met 1 Facility immediately placed the water outlet out of order and provided bottled drinking water to children. The faucet has been replaced, restested and no longer has an exceedance. Deficiency has been corrected. LPA Rubio received the facilities water testing results for Faucet B with an Action Level Exceedance higher than the allowable limit. 8

Based on LPA Wilburn's record review, the licensee did not comply with the section cited above in 1 out of 5 employees did not have proof of required immunization's at time of visit. Staff #5 did not have proof of immunization's (Measles, Mumps, Rubella and Pertussis), which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA Wilburn's record review, the licensee did not comply with the section cited above in 2 out of 5 persons, Director's mandated reporters certificates expired 01/20/2022 and Staff #5's record did not have proof of completion at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.
