… 1 Director stated she would hold a staff meeting with S1 and provide training on personal rights and provide proof of the completed training by 4/29/25 and e-mail to LPA Lane. Based upon investigative interviews, four individuals stated they witnessed S1 using inappropriate language in front of the daycare children multiple times, which is a potential health, safety and personal rights risk to children in care.
Based on LPA observation, staff Jaclyn Lyberger was observed supervising three toddlers children alone. Ms. Lyberger did not have units in Infant/Toddler on file at the center and is not a qualified teacher which poses/posed a potential health, safety or personal rights risk to children in care.
Based on observation, LPAs observed that site director, Coralito Garcia in the toddler classroom as facility did not have sufficient staff on site to provide the necessary needs of the Toddlers present today.

... 1 Director moved C2 back to the infant room after naptime and stated she would call the parent to ask them to wait until a space is available in the infant room. Director will create a letter to the parents recommending a solution, ask them to sign it and send to LPA by 1/30/25. C2 shall remain either in the infant room or on hold until available spot is available. Based upon LPA observation and record review, C2 was enrolled in the toddler component and was not yet 18 months old which is a potential health, safety and personal rights risk to children in care.
101227(a)(1) In child care centers providing meals to children...All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. 1 Licensee stated, food is now delivered on Fridays for the following week operations, instead of Mondays. Licensee will also provide a written plan of correction to the Department by 11/19/24 regarding how they will ensure there is adequate food supplies to meet children’s needs and menu on a consistent basis. Based on the information obtained during interviews, observations, and documentation reviewed it is determined that there is insufficient menu food to meet day care children’s needs. Children are often unable to have second servings of one or more menu items 2-3 times per week and food supplies frequently run very low by the end of the week, 8 resulting in children being fed whatever is available until the new food shipment arrives which poses/posed a potential health, safety or personal rights risk to children in care.
Based on LPA observation, staff Safia Azimi was observed supervising the toddlers children alone. Ms. Azimi has has only 3 ECE units on file at the center and is not a qualified teacher which poses/posed a potential health, safety or personal rights risk to children in care.
Based on the information obtained during interviews, observations, and documentation reviewed it is determined that child C1, under 24, months has been participating and comingling with 2’s preschool class and child C2 and C3, under 18 months, 8 have been participating and comingling with toddler classroom which poses an immediate health, safety or personal rights risk to children in care. Based on the information obtained during interviews it is determined that toilet paper was unavailable to children from 9/26 – 9/31/24 then sporadically afterward for periods of 1-2 days at a time. Staff would use paper towels or wet wipes from other children until 8 supplies were refreshed, which poses an immediate health, safety or personal rights risk to children in care.
Based on the information obtained during interviews and documentation reviewed it is determined Staff S1, supervised 8 preschool children alone, but is not qualified to supervise children alone which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on the information obtained during investigation interviews it is determined that staff S1 brought their school age child to the facility 2-4x per week over the past 2-3 months and remained in the preschool classroom for up to 8 hours which poses a potential health and safety risk to children in care.
Based on observation and interviews, Toddler room 2 walls had construction without notifiying the Department which poses a potential health and safety risk to children in care.

Director stated they will double the food ordered and provide proof of food ordered in accordance to the menu and enrollment and provide purchase order to the Department by 3/22/24 for the week of 3/18/24 -3/22/24. Based on interviews, observations, and documents reviewed LPA observed insufficient food for children during AM snack, investigation interviews support allegations, and purchase orders and delivery times show in February show insuffienct food at the begining of the week which poses/posed a potential health, safety or personal rights risk to persons in care. 8
Based on interviews, facility was not in good repair when the center opened due to flooding over the weekend which poses a potential health and safety risk to children in care.
Based on interviews, facility failed to inform the parent of the damage sustained at the facility over the weekend and subsequently failed to inform the Department of the damage and temporary closure from 2/5 - 2/7 as a result.
Immunizations: Prior to admission to a family day care home, children shall be immunized against diseases 1 Director has provided missing immunizations as of site visit. Deficiency is cleared. Based on records review, it was determined that S1 has been working at the facility today prior to obtaining a criminal record clearance, which poses and immediate health and safety risk to the persons in care.
Based on interview with Director and a review of Guardian and the Licensing Information System, A+ sub, Coralito "Coral" Garcia has been working for the facility for three weeks without a requested transfer. This is a potential risk to the health and safety of children in care.
Based on interviews and review of relevant documentation, Staff #1 was asleep, on at least one occasion, while supervising napping children, creating an immediate hazard to children in care. 8 Director and Assistant Director Kinsey McCabe state that they were not aware of the situation prior to this visit and therefore, unable to address or put preventative measures in place.
Based on review of relevant audio documentation and interviews, Staff #1 spoke harshly to children, causing them to cry and Staff #2 spoke in a demeaning manner to a child on at least one occasion. Based on interviews, Teachers have been watching each other's classroom, with a combined total of more than 12 children, while one or the other steps out. This is a potential hazard to the health and safety of children in care.
